The game only gets darker and darker from there, eventually falling apart completely and revealing yet another layer to the puzzle. You battle your mother as a boss, throwing bombs at her eyes until she explodes in a shower of blood. If you kill enough angels, you can grab a key and battle Satan himself as the final boss. At first glance, you’d never guess DDLC of being anything other than a cute dating sim with an adorable cast of hot anime girls – but you should never judge a book by its cover.

Some Starfield players — including me — are experiencing a strange audio bug that causes missing dialogue and sound effects alongside echoed sound distortion. The issues appear to be impacting Starfield on Xbox Series X. Detention is really that kind of game that forces you to draw a line between “scary” and “disturbing.” Parts of Detention are certainly scary in the traditional sense, but this isn’t really the kind of game you play when you want a good jump scare. Instead, Detention is the kind of game you play when you want to test the limits of your ability to process some heavy and unnerving material.
Hellblade: Senua’s Sacrifice is lauded by critics and mental health specialists for its realistic portrayal of mental illness, and the distortions in perceived reality experienced by psychotic patients. Who are we kidding — there's no way Pony Island is disturbing, right? Just a nice relaxing game about ponies chilling on an island, eating ... oats? Is that what ponies eat? The game is basically a mass-murder simulator where you rebel against mankind’s inherent evil by killing as many innocent NY citizens as you can.
While it does start as your average high school romantic comedy where a guy joins the Literature Club to keep it from being closed down, things go south fast once the girls start showing their true (and disturbing) selves. I know I just used that “snuff film” comparison an entry ago, but it’s kind of hard to talk about Manhunt and not bring that idea up again. After all, the premise of this game revolves around the idea of entertaining a twisted “director” who isn’t satisfied with the murders you commit unless they’re as violent as possible. I could see some arguing that The Cat Lady isn’t a “horror” game in the most traditional sense of the idea, but I truly believe it qualifies for the purposes of this list. Besides, few will argue that this game is anything less than seriously troubling, regardless of which genre they ultimately put it in.Despite riding a pretty sizeable wave of hype for such a small game, Agony turned out to be a pretty disappointing overall experience. However, I’ve yet to meet a person who has played this 2018 title and doesn’t consider it one of the most disturbing games ever made. That’s exactly the kind of world explored in LISA: The Painful, where your balding middle-aged playable character must protect his adoptive daughter from the dangers of a post-apocalyptic world with no women. This is a list of disturbing horror games rather than disturbing games. The line between those types of games can be fine, but the basic idea is that this list doesn’t include otherwise non-scary games with disturbing moments.
